… 1 Fence was fixed today while LPA was present. Director understands that she must check on fence periodcally to make sure planks are attached and fence is in safe condition. Based upon facility interviews with multiple parties, the facility did not fix the broken fence upon notification which poses a potential health, safety and personal rights risk to children in care.

Licensee states that she disagrees with the deficiency and that access to the file and requested copies of documents were provided. Licensee will be appealing the citation. Based upon interviews, the facililty, denied the parent access to Child #1's file upon disenrollment. This is a potential risk to the health and safety of children in care.

Director has already submitted the incident report and will ensure that any unusual incident that is a threat to the health and safety of children in care will be timely reported to Licensing. The facility did not report a power outage that occurred on 10/28/22, lasting from approximately noon to closing which could pose a threat to the health and safety of children in care should the power outage cause disruption in care or the inability to provide and meet the needs of all children during the outage.
