Based on observations, interviews, and record reviews, facility staff did not provide adequate supervision which resulted in a daycare child sustaining a fracture. This poses an immediate health, safety, or personal rights risk to persons in care. 8 This deficiency shall be cited and cleared on this day.

Based on observation and record review, Licensee did not comply with the above by not ensuring that a staff member (S1) was associated to the facility prior to their presence in the facility. This poses an immediate health, safety, or personal rights risk to persons in care. 8 This deficiency shall be cited and cleared on this day, 7/2/2024.

Based on observation, interview, and record review, Licensee did not comply with the above by allowing children under the age of three to use play equipment intended for use by children age three and up.
