Licensee/Program Director agreed to submit a plan of correction outlining the measures on how to ensure that adequate supervision is provided to the children. POC must be submitted no later than 9/19/2024. Child 1 managed to cross over the fenced playyard and walked/ran 97 feet away from the fence (by rolling two tires near the fence and stacking them together) unnoticed by the staff. This poses an immediate risk to health and safety of children in care.

Program Director agreed to submit a plan of correction outlining the measure how to meet the four-foot fence requirement. POC must be submitted no later than 9/27/2024. LPA observed and measured the fence. confirming that it stood 3.5 feet in height which is below the regulatory requirement of four (4) feet contributing to C1's ability to climb the fence. This poses a potential risk to health and safety code of children in care.

The Site Supervisor and Director agreed to submit a written plan of correction outlining the measures on to how ensure that any reportable incident will be reported to the department within 24 hours of its occurrence. POC shall be submitted on or before than 9/27/2024. The Child Care Center failed to report the incident of a child that jumped over the play yard fence outside the designated fenced paly yard. This poses a potential risk to health and safety of children in care.

The aforementioned to be submitted to CCLD (elvin.baddley@dss.ca.gov) by the close (5:00 PM) on 9/12/24. did not notify a parent removing C1 from care on 8/9/24, C1 was struck by a teacher in care earlier in the day of 8/9/24, despite S2 witnessing the incident. This poses a potential risk to health, safety or personal rights of persons in care.
