Based on record review and interview, the licensee did not comply with the section cited above. The facility has not completed the required water testing for lead contamination levels. Water lead testing was to be completed by January 1, 2023.

Based on observation, the licensee did not comply with the section cited above. LPAs observed several disinfectants, cleaning solutions, and paint containers were accessible in the staff bathroom by door being unlocked, which poses a potential health, safety or personal rights risk to persons in care.

Based on records reviewed and interviews, two files out of four were missing documents. Staff #2 and Staff #3 were missing measles records, which poses a potential threat to the health and safety children in care.
