(a) The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors. This requirement was not met as evidence by: 1 Wood planks were removed, wires in the nap room were made inaccessible and carpet was removed during inspection. New carpets were purchased and put in place during inspection. Based on observation the licensee did not ensure the facility was clean, safe and sanitary. This poses a potential Personal Rights risk to children in care. 8
(b) Any unusual behavior, any injury or signs of illness requiring assessment and/or administration of first aid by staff shall be reported to the child's authorized representative and recorded in the child's record. This requirement was not met as evidence by: 1 Per Director, they are currently using the Bright Wheel Application to inform parents of any incident or injury that occur at the school. Facility also utiilized written reports that parents have to sign. Director will ensure she gets a respond from parents confirming receipt of message. Based on record review the licensee did not ensure parent was notified about an injury of their child. This poses a potential Health, Safety or Personal Rights risk to children in care. 8
Based on the licensee did not comply with the section cited above. LPA observed multiple flies in the outdoor space in trash cans, dumpster and throughout the yard, which poses an immediate health, safety or personal rights risk to persons in care.
Based on observation, the licensee did not comply with the section cited above. LPA observed large pot holes outside in the cement area where the bikes are used and a tree stump with sharp wood sticking out, which poses an immediate health, safety or personal rights risk to persons in care.

LPA observed broken picnic tables, ripped child's couch, cracked child's plastic chair, rusted and broken bikes and a broken baseboard with exposed staples, which poses an immediate health, safety or personal rights risk to persons in care.
