Based on observation, interview, record review, the licensee did not comply with the section cited above in2 out of 4 children did not have their 15 minute safe sleep logs in their file for LPA to review, which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on observation, interview, record review, the licensee did not comply with the section cited above in1 out of 3 staff did not have proof of measles and pertussis immunization, which poses/posed a potential health, safety or personal rights risk to persons in care.

Director of operations will stop commingling the toddlers from both licenses. Director will also provide a declaration stating that they understand that they cannot commingle with toddlers from a different license. as toddlers from #198006504 were commingling with toddlers from the above facility during outdoor play time causing the facility to go over capacity.

Based on interviews conducted and observations, although there were staff on the playground and aid provided immediately, staff did not witness the actual impact of the C1 getting hurt.

Facility will write a declaration stating that they understand and will follow title 22 reporting requirements. Facility did not report the unusual incident report in a timely manner as the incident occurred on 9/12/2025 and facility reported the incident to the department via email on 9/16/2025
