Based on interviews and LPAs observation the Center did not ensure each child had an individual storage space for personal belongings when LPA observed children’s clothing and diapers for multiple children in one storage space on January 23, 2025.
Based on LPA observation and interviews the center was not clean, safe 1 Per facility representative, the Center completed a staff training regarding cleanliness and maintining a clean and healthy environment on March 6, 2025. A copy of the training was received. and sanitary as the toys in the classroom had brown dirt marks from use, the potty-training seats left marks on toilets seats and paint was peeling and chipping off the walls in a classroom. This poses a potential Health, Safety, or Personal Rights risk to persons in care. 8 The Center created a daily checklist for the staff to utilize which are turned in weekly for management to review.
Based on record review , the licensee did not ensure 1 Per facility representative, a staff training was held on 1.29.25 which addressed staffs interactions with children. each child was accorded dignity when S2 told the classroom that monsters were in the closet as a means of stopping the children from opening the closet door in the classroom. This poses a potential Health, Safety, or Personal Rights risk to persons in care.
Based on interviews and 1 Per facility representative the staff completed a training on January 29, 2025 regarding teacher interactions. and an active supervision January 16, 2025. A copy of the training was received. observations, teachers are responsible for cleanliness of the classroom. In order to complete this task, the teachers are performing housekeeping duties while providing care and supervision. This poses a potential Health, Safety, or Personal Rights risk to persons in care.
Based on record review, the licensee did not ensure each child was accorded dignity when S1 made threatening comments regarding police contact to C1. This poses a potential Health, Safety, or Personal Rights risk to persons in care.
Based on record review, the child care center is a Title 5 program therefore the policies and procedures must comply AB 2806. A NOA was not issued at least 24 hours prior to C1s termination. This poses a potential Health, Safety, or Personal Rights risk to persons in care. 8 In addition, per facility representative the Center recognized the mistake with the written notification and within 24 hours offered to extend care.
Based on observation, the licensee did not comply with the section cited above when Staff 1 was supervising 16 children, which poses an immediate health, safety or personal rights risk to persons in care.
