Based on interviews and photos, it was determined that the facility had a cat that would urinate and defecate in the classrooms. Based on interviews and photos it has been determined that thre was a water leak in the Pinecones class and leaked down to the Sprouts classroom. This posed a potential risk to the health, safety, and personal rights of children in care.
Based on record review, the licensee did not comply with the section cited above in 2 out of 6 childrens files contained medical consent forms with missing signatures which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on interviews conducted, it was determined that staff failed to provide adequate supervision resulting in children hitting and injuring each other. This poses an immediate risk to the health, safety, and personal rights of the children in care.
Based on interview and record review, the licensee did not comply with the section cited above by ensuring all staff have required immunization records in file, which poses/posed a potential health, safety or personal rights risk to persons in care. 2 staff are missing proof of immunity for Measles and Pertussis.
