Based on the observation, interview, and record review, on 2/8/2023, the facility did not properly supervise Child 1 and Child 2 which resulted in inappropriate contact between the children which poses an immediate health, safety, or personal rights risk to persons in care. A $500 civil penalty is being assessed as a result of neglect and lack of supervision.

Based on the observation, interview, and record review, the facility did not meet the requirement when they did not report the incident that occurred between Child 2 and Child 3 which poses a potential health, safety, or personal rights risk to persons in care.

Based on the observation, interview, and record review, the Licensee did not meet the needs of Child 2, on multiple occasions Child 2 had behavioral issues with hitting, biting, and outburst and the facility did not have staff trained in meeting needs of children with behavioral issues which poses a potential health, safety, or personal rights risk to persons in care.
