(a) A licensee shall not operate a child care center beyond the conditions and limitations specified on the license, including the capacity limitation. This requirement was not met as evidence by: 1 Per Facility Representative, they will work with enrollment for notifications on documentation stating our limitations of capacity and to plan in advance. Also to verify consistent families who will be attending on any given day. Staff training will be conducted regarding capacity adherence Based on interview and record review, Licensee did not ensure that the licensed capacity of 80 was maintained when there were at least 84 children on site during a field trip day 06/30/2026. This was an immediate risk to the health, safety, and personal rights of children in care. 8 during summer sessions, field trips, and school year combination days. COmmunication of head counts throughout the day will be trained. Training agenda with signatures, email to enrollment, and updated field trip permision slip notations will be sent to LPA by POC date

Based on record review and interview, licensee did not ensure field trip provisions were met when Child #1 was taken on the field trip without permission/consent from authorized representative. This was a potential risk to the health, safety, and personal rights of children in care 8 and headcount for field trips. Children will be grouped off beforehand once they are signed up, so the day of, if there is a child that is not on the list it will raise an alert to double check. This will be drafted as a training/memo and sent to LPA by POC date.

No child(ren) shall be left without the supervision of a teacher at any time... Supervision shall include visual observation. This requirement is not met as evidence by: 1 Per Facility Representative, they will conducted supervision training with all staff, and create an agenda all staff in attendance will sign. Training will include topic of having children move with staff member, when on their own overseeing group outside. Based on interview, Licensee did not ensure children were supervised visually at all times, as two children were out of view of Staff #1 for an unknown amount of time, while outside. This posed a potential risk to the health, safety, and personal rights of children in care. 8 They will submit an updated staff schedule to help provide assistance and ensure staff are not alone with a group, when possible, other than the end of day. All corrections will be submitted to LPA by POC date.
