Based on interview and record review, the licensee did not comply with the section cited above in 2 out of 2 staff. Staff #1 and Staff #2 did not have current updated Mandated Reporter training certificate which posed a potential health, safety or personal rights risk to persons in care.

(a) The licensee shall ensure that personnel records are maintained on the licensee, administrator and each employee. Each personnel record shall contain the following information: (11) A health screening as specified in Section 101216(g).This requirement is not met: 1 Director stated that she will send in the health screening for S2 and S3 by Oct 16, 2021. Based on record review, S2, S3 are missing health screening. This poses a potential Health and Safety risk to the children in care. 8
