Based on observation and interview, the swimming pool is being rented by an 1 Licensee Representative Ward stated they will cease operation of Phoenix Swim Academy and a document will be provided to the LPA. outside company Phoenix Swim Academy during operating hours and there is no approval by the Department of such use. This is an immediate risk to health and safety of children in care. 8 Request Denied
Licensee Representative Ward stated they will remove and take down the fencing immediately and remove the bicycle lock. Licensee Representative Ward they will remove the door and make it a drywall to make it inaccessible to children in care. LPA observed there is a bamboo fencing obstructing the view of the pool. Room #6 has a side door which lead directly to the swimming pool and is accessible to children in care. This is an immediate risk to health and safety of children in care.
Based on observation, 1 infant and 2 toddlers were comingling and all napping at room #1 with 2 staff. This poses a potential risk to health and safety of children in care.
