Based on observation of staff records. LPAs observed that Staff #1 and Staff #2 did not have required immunizations on file. Director did not have a influenza declination statement on file. This poses a potential health, safety or personal rights risk to persons in care.

Based on observation of staff records. LPAs observed that staff did not have documentation of a current AB 1207 Mandated Reporter certification on file. This poses a potential health, safety or personal rights risk to persons in care.

Based on file review and statement from Director. During inspection Director stated that she has been the Director for nine years. A review of facility file shows no record of required paperwork provided by Licensee indicating her as Director. Community Care Licensing records indicate a different Director on file. This poses a potential health, safety or personal rights risk to persons in care.

interviews and observation. Multiple interviews revealed that 1 Director stated that a statement would be completed by the facility representative including how the facility will ensure they are up to date on the most current California Department of Public Health (CDPH) mandates and gudiance for child care facilities and how they will implement the requirements in their facility at all times. Proof will be submitted by POC date 3/21/2022. the facility allowed parents and staff to enter the facility withought a mask on multiple occassions and interviews and observation revealved that the facility did not follow the CDPH requirements on mask wearing for children in care. This poses a potential risk to the health, safety and/or personal rights to children in care.

record review and interview. 1 Administrator Patrick Dodd removed staff #1 today from the preschool classroom and placed a Department cleared staff (staff #2) to stay within ratio. Today, staff #1 was sent to get the proper clearance and will not return to the facility until she/he receives proof of Department clearance. Staff #1 obtained criminal record clearance through his/her employer but not the Department of Social Services. According to records review, staff #1 does not have a CACI clearance. Administrator confirmed this information. A civil penalty of 100.00 was assessed today. 8 Proof of stamped Live Scan form will be provided by POC date: 1/28/2022. Deficiency will be cleared upon staff #1 obtaining all proper clearances (DOJ, FBI and CACI) and providing proof to the Department.
