This requirement is not met as evidence by: During a visit on 4/27/22, the Director engaged in conduct inimical to the health, welfare, and safety of children in care or the people of this state by not being forthcoming with the Department in regarding to arrangement made with the parents of infants being cared in unauthorized rooms. Director permitted the relative to continue to provide care & supervision to an Infant in the unauthorized room. In addition, the director was argumentative and hostile towards the Department’s Representatives. This poses an immediate risk to safety of the children in care. 8

This requirement is not met as evidence by: 1 The Director stated did not agree of the report and will be appealing the report. During the investigation on 4/27/22, the Director engaged in conduct inimical to the health, welfare, and safety of children in care or the people of this state by not being truthful with the Department in regarding to arrangement made with the parents of infant being cared for at the church area. In addition, the director yelled at the Department Staff multiple times. This poses an immediate risk to safety of the children in care. 8
Based on interviews, LPAs learned that adult #1 & Adult#2 & Adult #3 who disclosed the Licensee was operating in the Youth Room and infant room. During the Prelicensing inspection the Department Analyst inspected Room #1, #2, #3, & #4, the Youth Room and infant room was not inspected by the 8 Department and a Fire Clearance was not obtained for the Youth Room and infant room therefore the Licensee is not authorized to utilize the Youth Room and the Infant Room for the day-care facility. This poses an immediate safety risk to children in care. The facility is cited for repeated of violation, Pending civil penalty.
Based on observation and interviews conducted, it was revealed 6 children who are infants under 18 months old were being taken care of under the toddler option in preschool license with children ages 18 month and 36 months which poses an immediate risk to the health and safety of the children in care.
Based on interview and record review, the facility did not comply with the section cited above; staff members did not have mandated reporter training certificate which poses a potential health, safety or personal rights risk to persons in care.
