Based on (interview) and (record review)], the licensee did not comply with the section cited above by ensuring at least one staff member, who has valid CPR/First Aid certification, is present when children are at the child care center, which poses/posed a potential health, safety or personal rights risk to persons in care.
Licensee shall maintain a lead value at or below the Action Level of 5 ppb in all outlets subject to the testing requirements of these Written Directives, for the health and safety of children in care. This requirement is not met as evidence by: 1 The facility placed both outlets offline and made them inoperable. By 08/18/2023 LPA will follow up with the facility to see if the fixtures will be repaired or permanently removed. Based on record review the licensee did not comply with the section cited above as outlets Library Sink 1 BLDG 2 and Bubbler BLDG 6 Sink 1 exceeded the acceptable amount of lead allowed in a child care center, which poses a potential Health and Safety risk to persons in care. 8
