This poses an immediate health, safety and personal right risk to the children in care. 8 adequate supervision, specially at the exit areas. A work order will be placed for install permanent two gates on the playground for additional supervision and safety. Safety will be reviewed with children. A written declaration will be submitted to LPA by 07/01/25.

Per Program Supervisor two water faucets in room 1107 currently not in use and both water faucets were disconnected since Covid 19 started, 03/2020. Per Program Supervisor the facility was not using two water faucets in room 1107 even prior to the discovery of lead level exceedance. The facility was inspected and water samples were collected on 04/12/23. Sample Site D, two water faucets in Room 1107 were reported with 15.300 ppb or greater lead exceedance levels. This poses a potential health, safety and personal right risk to the children in care. 8 The two water faucets were observed to be disconnected. Per Program Supervisor, they will have parts of faucets replaced and obtain a retest of it afterwards.

Based on LPA's observations and Program Supervisor confirmation, the licensee did not comply with the section cited above and LPA observed two big trees roots which are located on the outdoor playarea across from the administrative office are enough out of the ground which can be tripping hazard for children which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on Program Supervisor and LPA's observations Room 34 currently is used by School District Special Need Preschoolers and Room 35 is used by school age children, however Room 34 and Room 35 are licensed for this preschool program , the licensee did not comply with the section cited above and did not report classrooms changes to Licensing in a timely manner which poses/posed a potential health, safety or personal rights risk to persons in care.
