Responsibility for Providing Care and Supervision: The licensee shall provide care and supervision as necessary to meet the children's needs. No child(ren) shall be left without the supervision of a teacher at any time, ..... Supervision shall include visual observation. Licensee is to create a plan to ensure that there are no blind spots on the playground. Staff are to be trained on active supervision. Training shall also include proper steps and procedures to follow should an altercation occur between children. Plan to be submitted to LPA no later than 5/20/2026 via email. This requirement is not being met as evidence by: Interviews conducted stated that an incident had occurred that while staff were present, they did not observe the incident occur which resulted in a child sustaining a deep scratch by another child. This poses a potential health and safety risk to children in care. 8

Licensee is to provide a signed written statement that Licensee understands and agrees to follow Title 22 Section 101219. Plan of topics discussed as well as signatures of attendees to be submitted to LPA no later than 11/28/2022. This requirement is not being met as evidence by: Based on interviews it was determined that there have been multiple incidents of children hitting/throwing items at other children and staff. Although staff were present during these incidents, staff did not provide the adequate supervision to prevent the continual incidents from happening. This poses a potential health & safety risk to children in care. 8

A complete Director packet is to be provided to LPA within 30 days (by 10/8/2022). This requirement is not been met as evidence by: Based on the LPA record review and staff interviews, the facility does not have a current Qualified Director.
