Based on interviews staff S1's placement of leg over child during nap time when child would not stay still posed a potential Health, Safety or Personal Rights risks to persons in care. 8 for staff training on all safety procedures and protocols. As well as implementation of a second staff member in the room, when needed, to accompany a child who is having difficulty with rest time.
Based on record review and interview with Director, the licensee did not comply with the section cited above in 8 out of 8 staff records which poses a potential health, safety or personal rights risk to persons in care.
Based on water testing results and interviews, facility tested over the Action Level Exceedance level for the drinking fountain outside women's restroom near the sanctuary which may have been used for drinking prior to the lead exceedance which posed a potential health, safety or personal rights risk to children in care.
