interviews and record review. 1 Today, Site Supervisor Cervantes stated the facility had since obtained criminal record clearance for both staff #1 and staff #2 as required which LPA was able to confirm through the online Guardian website. LPA also collected a copy of livescan forms for staff #1 and #2. An Investigation revealed that staff #1 and staff #2 were present at the facility (as early as December 2022) on a daily basis for several hours a day and were providing care and supervsion to children without criminal record clearance. This poses an immediate risk to the health, safety and/personal rights risk to children in care. 8 Site Supervisor stated she would also provide a written signed and dated statement indicating the understanding of the requirement to obtain criminal record clearance for adults who work and provide care and supervision to children. Proof will be submitted to the Department by POC date 3/1/2023.

Deficiency was cleared today.

Interview and Record review. 1 Site Supervisor stated she would provide training to staff on this requirement and submit proof of the training including a dated agenda with topics reviewed signed by each staff. record review and interview. 1 On 2/29/2023, Site Supervisor provided proof that she had completed the online record keeping orientation. Site Supervisor also completed an children's roster which was provided to LPA. Licensee stated after taking the online orientation, she understands the requirement to have a completed children's roster as well as the understanding of how to manage and maintain records. Deficiency cleared today. On 2/8/2023, Site Supervisor could not provide the Department with current or past Children's Roster that contained all required information. This posed a potential risk to the health, safety and or personal rights to children in care.
