Based on a self reported UIR and staff interviews, it has been determined that staff did not ensure children were provided visual supervision at all times while using exiting the classroom to the cafeteria for pick up time, which poses an immediate health and safety risk to children in care. Facility representative states and LPA verified on visit today that children are now picked up from the classroom and no longer walk to the cafeteria for pick up.
