Water bugs were observed in the infant’s room, which poses a potential risk to the health and safety of children in care. Pest control services are scheduled twice a month. The plastic hose hanging from the ceiling in the babies' room had been removed, AC issue was resolved, and staff confirmed that there had been no water bugs since the removal.

a sleeping infant was rolled out of the crib area in their crib to the outdoor area. 1 A staff meeting was held on 06/14/2024, Infant Safe Sleep was discussed. Extra TA is in the classroom to visually supervise infants in the activity area and in the crib area. Staff understand that infants must sleep in their crib inside the crib room. A copy of the meeting agenda was provided. The staff admitted that the infant’s sleep was disturbed

Based on record review, the licensee did not comply with the section cited above following a file review and per site director Georgina Garcia Figueroa, the previous director was reassigned and Georgina has been acting director for 2 years currently, which posed a potential safety risk to persons in care.
