Copy of personal rights will be given to staff, examples of type A citations. Staff will sign form to confirm handout was proviced. Meeting will be conduct beginning 06/12/26 until 06/30/2026. Per video footage reviewed, Staff stated child ran out of the line and was brought back to the classroom and waited until child was calm before taking child outside. This action is a violation of a child’s personal rights and is poses an immediate risk to the health and safety of the children in care. 8

Copy of handouts will be provided andl sign in sheets by 06/30/2026 Per video footage reviewed two children were left unattended while one staff member was in the bathroom changing a diaper and was unaware of the incident. This action is a violation of a child’s personal rights and is poses an immediate risk to the health and safety of the children in care. 8
