(d) Upon the occurrence, during the operation of the child care center of any of the events specified in (d)(1) below, a report shall be made to the Department by telephone or fax within the Department's next working day and during its normal business hours. 1 LPA received a completed Incident Report on today's visit. Deficiency corrected and cleared on today's visit. In addition, a written report containing the information specified in (d)(2) below shall be submitted...This requirement was not met as evidence by; Facility did not report the incident ocurred on 06/13/22 to the Depatment. This can pose a health and safety risk to the children in care.

101216.1 (b)(1) Teacher Qualifications and Duties(b) Prior to employment, a teacher shall meet the requirements of (b)(1) or (b)(2) below: (1) A teacher shall have completed, with passing grades, at least six postsecondary semester or equivalent quarter units of the education requirement specified...This requirement was not met Director stated that she will make sure that there is always a fully qualified teacher scheduled in every classroom. Also Director stated that if necessary she will help to substitute teachers during absence for lunch/breaks. as evident by;LPA found out through interviews that a teacher assistant was left alone in the classroom supervising 8 children. Teacher assistant did not have the ECE units required to be a qualified teacher. This is a requirement that if not corrected pose a health and safety risk to the chidlren in care. 8 LPA Sierra received a written plan of correction during today's visit and also staff/classroom schedule. Deficiency cleared and corrected during today's visit.
