The facility took steps to remediate the fixtures by providing an alternative water source that did not have an exceedance and retesting the fixture on 3/31/2023. The fixtures then tested below the ALE level and are now in compliance. Based upon record review, the facility had one fixture (R) that tested above the Action Level of 5ppb. This posed a potential health, safety, or personal rights risk to persons in care. 8 This deficiency will be cited and cleared the same day of the visit, 11/7/2023.
