The Department has the authority to approve or deny a facility license, or employment, residence or presence in the facility, based on the results of this review. 1 Director associated S1 during today's visit. Director to review Guardian roster process with administrative staff and create a plan that addresses all new staff people to ensure any new personnel is background checked and associated prior to working at the facility. This requirement has not been met as by: S1 was not associated to the Facility Roster on Guardian. This poses a potential risk to the health, safety and personal rights of children in care. 8

Outdoor Activity Space: The playground shall be enclosed by a fence to protect children and to keep them in the outdoor activity area. The fence shall be at least four feet high. This requirement was not met as evidence by; 1 Site Director Danna Osborn stated she is going to replace the gate with a security gate that is more than 4ft by August 26, 2024. She will send a photo to licensing when this is completed. LPAs observed the fence surrounding the playground was over 4 feet, however the gate was shorter than 4 feet. This posses a potential risk to the health, safety and personal rights of children in care. 8
Buildings and Grounds: All licensees shall ensure the inaccessibility of pools, including swimming pools (in-ground and above-ground), fixed-in-place wading pools, hot tubs, spas, fish ponds or similar bodies of water, through a pool cover or by surrounding the pool with a fence. 1 Site Director emptied the water from tables and buckets during the inspection. Deficiency corrected. The above requirement was not met as evidence by: LPAs observed standing water in the water table, two buckets filled with water on the other side of at least a 2ft fence. This posses a potential risk to the health, safety and personal rights of children in care. 8
Staff immediatley moved cleaning products, and director to send documentation of training for staff. During tour of facility at 3:25pm it was observed in classroom 2 that facility did not ensure that cleaning products were locked and made inaccesible to children. This posed an immediate risk to children in care
Staff that initiated the incident was terminated, director to conduct staff training 4/18 to discuss conduct in front of chidren. Director to send outline for meeting as proof of correction. Based off of interview of staff and management it was stated that staff did engage in a verbal altercation 3/31/2023 while children and parents were present. This poses a potential risk to children in care.
Based on observation and record review, the licensee did not comply with the section cited above in 4 out of 4 staff files were missing documentation of immunizations which poses a potential health, safety or personal rights risk to persons in care.
