Based on interviews, the facility did not comply with the section cited above where children were not properly supervised resulting in a child being left on a playground unattended, which poses an immediate health, safety or personal rights risk to persons in care. 8 Facility will produce a zoning map plan and post in all classrooms. Director will submit signed meeting notes and as well copies of walky talky log and zoning map to LPA for one week post meeting. ABSENCE OF SUPERVISION AND REPEAT CIVIL PENALTIES ASSESSED

LPA provided copies of LIC624 for facility to use. Facility will send reports for incidents on 4/26/24, 2/9/24 and 6/23/23. Director stated they will call the officer of the day or LPA with any questions regarding if an incident needs to be reported or not Facility did not inform CCLD of the unusual incident that occurrent, within 24 hours, nor did she submit a LIC624 within 7 days. This is a potential health and sfety risk to children in care.
