Based on LPAs observation and record review, LPAs found that there is not enough substrates under swings, slides, and climbling structures to absorb a fall. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

BUILDINGS AND GROUNDS: The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors. 1 Director stated that the chicken coop will be removed and the work order for the fence replacement has been placed. This requirement is not met as evidenced: There is a hole with protruding wiring in the chicken coop where a child could potentially get hurt. The outdoor chain link fence is hanging down and a large portion is missing due to recent fire. This is potential risk to the health and safety of children in care. 8
