Based on observation, the licensee did not comply with the section cited above in classrooms 2 with cleaners, toxic substances, rodent droppings, mouse traps, mops with solution next to and under sink area covered with curtain. Classroom 1 had Drano and toilet bowl cleaner under sink with a broken child lock, which poses a potential health, safety or personal rights risk to persons in care.
Based on interview and record review, the licensee did not comply with the section cited above in 3 out of 4 staff files reviewed did not have valid mandated reporter training, which poses a potential health, safety or personal rights risk to persons in care.
Based on observation, interview, and record review, the licensee did not comply with the section cited above by Director/Licnsee being out of facility for more than 30 days, starting 11/20/24 and scheduled to return 12/29/24 without a qualified substitute which poses/posed a potential health, safety or personal rights risk to persons in care.

The owner states they have staff who are not present today. Fix the schedule of staff and not use herself in the raito. Will submit new schedule (LIC500) to LPA Taylor at: andrea.taylor@dss.ca.gov LPA AND LPM observed staff go inside the front door and no staff on the playground with the children. Staff came back outside in less than one minutes time. This poses a potential risk to the Health and Safety of children in care.
The owner stated they will change the schedule of the staff and submit a new LIC500. The sign in and sign out sheets outside along with anything else they may use outside so they do not go inside. LPA AND LPM observed staff go inside the front door and no staff on the playground with the children. Staff came back outside in less than one minutes time. This poses a potential risk to the Health and Safety of children in care.
The owner will discuss with child's parent not allowed to have the infant child in the same area ss the preschool children. LPA and LPM observed a child under two years of age on the playground with the preschool children. This poses a potential risk to the Health and Safety of children in care.
The owner showed LPA and LPM invoice for pest control. She will clean out the files and see if there are any new droppings. The service is coming every 3 months Owner will submit all invoices for the year to LPA. LPA and LPM observed mouse droppings in the files which were reviewed during this inspection. This poses a potential risk to the Health and Safety of children in care.
LPA and LPM observed children's napping cotts with sheets on them stacked on top of each other allowing the bedding to touch This poses a potential risk to the Health and Safety of children in care.
