Responsibility for Providing Care and Supervision (a) (1) No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. This requirement was not met as evidenced 1 Site Supervisor provided documentation that a training on active supervision, staff responsibilities, and updated procedures for keeping account of children present, along with staff attendance sign in sheet, was completed at the facility. Site Supervisor also removed the blind spot in Room 4 and by interview and observation of video footage. Information obtained indicated that child #1 was left in Room 4 unsupervised for approximately 30 seconds. This poses an immediate risk to the health, safety or personal rights of children in care. A civil penalty of $500 was assessed. 8 has provided training materials for a future training to be conducted with all staff on 10/12/2026. Deficiency cleared.

Based on record review, the director did not comply with the section cited above by verifying staff is fully qualifed to provide care &supervision. This poses a potential risk to the health , safety or personal rights of children in care. 8 all regulatory requirements for teacher/teacher aide requirements and ratios for all classrooms and will submit the staffing plan to CCLD by 07/23/2025. Director Esparza will also submit exception requests for staff that are currently pursuing teacher qaulifications(12 ECE units).

, during today's visit, the LPA was made aware of staff #1 and staff #2 1 The director agrees to conduct a training with her qualified teachers, addressing covering classrooms while at the same time staying within the requirements. A plan will be made on how the director will supervise this. supervising 9 children by themselves. Neither staff have the required qualifications to meet the teacher requirements. This could pose a risk to children in care. 8 The training outline and sign in sheet will be sent to the LPA by 12/1/22.
