Facility site supervisor will have all staff review, sign and obtain a copy of their employee handbook, submit acknowledgment statements fromm all staff regarding the facility's fingernail length policy. Facility will also be conducting a re-training with all staff regarding the well being of children and their peronal rights. This requirement was not met as evidence by Child 1 being scratched by a staff member which poses an immediate risk to the child’s personal rights and physical safety. 8 Declarations will be submitted by all staff regarding what they learned from their training. Signed copy of all employee handbook signatures from all staff to be submitted to LPA by POC date.

Facility has terminated the staff member involved. Training has been conducted by facility on 2/13/2025 with all staff regarding personal rights and the well being of children/ peronal rights using CCLD videos on website. Documentation of the training will be submitted to LPA Perry by POC date. This requirement was not met as evidence by video review, staff interviews, and observations, it was determined that a S#1 physically handled C#1 in a manner that violated the child’s personal rights. S#1 was observed grabbing C#1 forcefully by the arms and placing the child down on the floor in a forceful manner. S#1 also used a loud, yelling tone and placed foot over C#1's foot in a restrictive manner. 101212 Reporting Requirements (d) Upon the occurrence, during the operation of the child care center of any of the events specified in (d) (1) below, a report shall be made to the Department by telephone or fax within the Department's next working day and Facility Site supervisor Darlene and administrator Ana Gamino to retake the CCLD Orientation course and submit certificates to LPA once completed. A declaration will be submitted to LPA Perry including timely and complete submission during its normal business hours...... This requirement was not met as evidence by two incidents that occurred on January 27 and January 29, 2025 that were not reported to the child’s authorized representative or to the Department as required. The failure to report incidents poses a potential risk to the childrens health and safety while in care. 8 of Unusual Incident Reports (LIC 624) and when it is necessary to submit one.
Based on observations and interview, the facility did not comply with the section cited above, LPA did observe sink to have broken, sharp and pointed parts on the edges as well as 2 sinks taped off. LPA also observed mirror next to sink to have dust, dirt and debris accumulated on the glass and a combination of trapped moisture and which is a potential health and safety risk to the children. LPA did observe mold like spots on the mirror.
Based on observations and interview, the facility did not comply with the section cited above, LPA Perry observed climbing structure to have visible spider webs, chipped paint, rust and stairs to the climbing structure have a rubber protector with tape on top that is peeling off and making the stairs to be unsafe whiches poses a potential health, safety or personal rights risk to persons in care.
Based on observations, interviews and record review, the facility did not comply with the section cited above, S#2 was missing immunization records, S#3 had no education transcripts, S#4 was missing immunizations, Mandated Reporter and education transcripts nd S#5 was missing immunization records, education transcripts and LIC9108 which poses a potential health, safety or personal rights risk to persons in care.

Site Supervisor states a meeting will be held with all staff in regards to supervision. Site Supervisor states minutes of meeting will be provided to LPA. Minutes will be submitted to LPA by
disclosures that were made during staff interviews that corroborate with the allegation of unqualified staff being used to assist with children. It was found that the housekeeper and kitchen staff have been used on more than one occasion to help with the supervision and ratio of children. LPA also observed this on 04/04/22, the housekeeper was being used to assist with children both indoors and outdoors. 1 Lead Teacher states staff meeting will be held to notify all staff that housekeeper and kitchen worker can not be used in the teacher to child ratio. MInutes will be submitted to LPA by 04/05/22.
