Based on observation, Facility did not comply with the section cited above, as LPA observed a worn changing pad, which poses a potential health, safety, or personal rights risk to persons in care.
Facility Representative stated they will provide Type A licensing reports to parents/guardians of children enrolled and any newly enrolled children. Facility Representaive will email proof or Aknowledgment of reciept to LPA by
LPA provided assistance to Facility Representative to move staff into the classroom that do qualifiy. LPA observed proof that staff members are enrolled in required courses. Facility Representative will send exception request for staff to LPA. based on observations and record review, the facility did not comply with the section cited above as staff observed in the infant classrooms do not meet the qualifications, which poses an immediate health, safety, or personal rights risk to persons in care.

Based on observation LPAs observed 17 infants in a classroom with 4 staff, which poses an immediate health, safety, or personal rights risk to pesons in care. Director moved an additional staff member into the classroom. Deficiency cleared during inspection.
PERSONAL RIGHTS: The licensee shall ensure that each child is accorded the following personal rights: To be accorded safe, healthful and comfortable accommodations, furnishings and equipment to meet his/her needs. Bonnie Lee stated she will have an all staff meeting on 1/26/24 informing staff it will be required for all staff to double check infant clothing after changing diapers and before child being picked up making sure all clothing are clean. She will send LPA signed agenda on or before 1/30/24. This requirement was not met as when the parent picked up the child from the facility, the child's clothing had a poop stain on it in the back and the child's neck area was very wet. Pictures shows either the child's diaper was not changed causing the poop stain or the child was not changed into clean clothing after having poop diaper changed and child not cleaned up around the neck area prior to being picked up by parent, This is an immediate risk to a child. 8 Owner, Bonnie Lee, was informed if deficiency is not corrected by due date a civil penalty may be assessed.

The Owner and Driector stated they have had an all staff zoom meeting with all teachers and reminded them to always have the bottles up high where a crawling infant cannot grab a bottle and to double check This requirement was not met as an infant was accidentally given the wrong bottle. The child was crawling and grab a bottle that sitting next to a staff. This is an potential risk to a child. the names on the bottles before the infant are given a bottle. Owner talks about all center challenges at the bi-weekly zoom meetings. Owner submitted a written statement and sent to CCL Deficiency cleared at time of inspection.
Objects used by infants that are mouthed, shall be washed and disinfected at least daily, or more often if necessary. Such objects shall include, but not be limited to, toys and blankets. 1 Owner stated she has changed procedure. Each child has their own bucket of toys to play with during the day and the toys are sanitized at the end of each day. Deficiency Cleared. This requirement is not met as evidence by based on photos an infant was playing with a cloth toy and had it in its mouth and then given to another infant to play with within minutes before being sanitized. This is an immediate risk to a child. 8 Buckets are kept on the shelf in the infant Baby's breath classroom. LIC 9224 was given and discussed with owner at time of inspection. Request Denied
Infant Care Center Sleeping Equipment: Cribs shall be free from all loose articles and objects, including blankets and pillows. This requirement is not met 1 Owner stated she will remind staff at all staff meeting that pillows shall not be used in the crib. The large size pillow was removed from infant baby breathe classroom at time of inspection. as evidence by based on photos an infant was sleeping on it's back on a regular size bed pillow. This is an immediate risk to a child. 8 Deficiency Cleared. LIC 9224 was given and discussed with owner at time of inspection.
Fixtures, furniture, and equipment that have been banned or recalled by the United States Consumer Product Safety Commission shall not be used or on the facility’s premises 1 Owner removed the one lounger that was in the infant baby breaths classroom was removed at time of inspection. Deficiency cleared. This requirement is not met as evidence by based on photos the facility does have a baby lounger that was being used and has been recalled. This is an immediate risk to a child. 8 LIC 9224 was given and discussed at time of inspection.
Based on record review a teacher only had three units in the infant cherry blossom classroom. She was being used as a qualified teacher and she did not have the required 12 units. Staff number two in the infant baby's breath classroom only had 9 units of the 12 units needed, she was also being used as a qualified teacher. This is a potential risk to children.
Owner had no proof of the 15 minute check on infants during sleep tine. This is a potential risk to children.
