(C)Preschool (36 months to enrollment in kindergarten) - 1:8 adult-child ratio This evidence is not met by: 1 Adminstration team will review procedure and will ensure to reponse to classrooms' needs in a timely manner to meet teacher-child ratio. Based on interviews, there were some mornings, classroom did not meet teacher-child ratio. 8 A written plan of how facility will response to classroom's needs in a timely manner to meet ratio requirement will be submitted to Licensing by 09/08/2026.

(d) Upon the occurrence, during the operation of the child care center of any of the events specified in (d)(1) below, a report shall be made to the Department by telephone or fax within the Department's next working day and during its normal business hours. In addition, a written report containing the information specified in (d)(2) below shall be submitted to the Department within seven days following the occurrence of such event. (1) Events reported shall include the following: (C) Any unusual incident or child absence that threatens the physical or emotional health or safety of any child. This Requirement is not met evidence by: 1 Administrators will ensure to report to Licensing of any Unusual Incident by phone with in 24 hours and follow up with a written report within 7 business days. Based on information collected, children had to be evacuated to the park due to alarm testing that went off many times throughout the day. Based on incident report and interviews, a child was left alone in the hallway. Classroom staff did not know they had left a child in the hallway until another facility staff brought the child to the classroom. Child being left without supervision poses a potential health and safety risk to children in care. 8 Proof of training/meeting agenda and meeting attendance sheet shall be submitted to Licensing by 07/03/2026.
