101229 (a)(1)Responsibility for Providing Care and Supervision a) The licensee shall provide care and supervision as necessary to meet the children's needs. (1) No child(ren) shall be left without the supervision of a teacher at any time....... Supervision shall include visual observation. POC; Staff that was in care of the child was given a noticed and facility changed the schedule so not all the children go out at the same time and Director talked to each staff members regarding supervision. This requirement was not met as evidence by; Interviews revealed that child #1 was left unattended in the reception area on 04/16/24 and found by a daycare parent. This poses an inmediate risk to the health and safety of the children in care. 8

101212(d)(1)(C) Reporting Requirements (d)Upon the occurrence, during the operation of the child care center of any of the events specified in (d)(1) below, a report shall be made to the Department by telephone or fax within the Department's next business day........ In addition, a written.......shall be 1 POC; Director will submit the incident report to LPA by due date. submitted to the Department within seven days following the occurrence of such event. 1)Events reported shall include the following: .....child absence that threatens the physical or emotional health or safety of any child.This requirement was not met as evidence by; Facility failed to report incident ocurred on 04/16/24. This can pose a health and safety risk to the children in care.

(a) The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors. This requirement was not met as evidence by: POC; During today's inspection no hazards were observed on the playground. Wet leaves were not observed, no standing water on the playground and the wood box was repaired.. On inspection dated 01/19/24 LPA observed wet leaves, poor drainage and splintered wood on the ground coming from a rotted wood box located in the preschool playground. This is a violation that if not corrected can pose a risk to the health and safety of children in care. 8

Physical Plant-Facility staff did not maintain the facility in safe, clean and sanitary fashion.
