Based on the Director's own admission on 02/21/2025 C1 was left alone in a classroom without staff supervision for roughly one hour.This posed an immediate health and safety risk to the child(ren) in care. 8 D1 also stated she will submit a statement on how the facility will train / on board staff regarding supervision. Based on the Director's own admission on 05/13/24 child C1 and child C2 was left without staff supervision for approximately 5 minutes while they wandered out to the street from the big play yard. This posed an immediate health and safety risk to the child(ren) in care.

Based on record review, the facility had 1 faucet (faucet F) that exceeded that allowable levels of lead in the water (5.6 ppb). This is a potential health and safety risk to children in care. receive drinking water from bottled water and a water dispenser. The licensee will be replacing the faucet and additional pipe under the sink on 10/14/22, have retested three weeks later and send results to CCL by 11/15/22. kevin.oconnell@dss.ca.gov

Based on record review at 11:00am, on 6/15/22, the licensee did not comply with the section cited above as three staff files did not contain current Mandated Reporter certificates as they had expired in January, 2022. Director stated that they were not current. This poses a potential health, safety or personal rights risk to persons in care.
