Director to secure immunization records for Staff #5 (S5) along with a signed LIC9052 and provide proof to LPA by 11/20/2024. All other files were corrected during today's inspection. During record review, LPA observed two files missing Immunization records, one file missing mandated reporter certificate and one file missing educational records. One file was missing an Employee Rights disclosure. This poses a potential health and safety risk to children in care.

Based on observation, LPA observed that the staff on the 3's playground were by the bouncy house. There were children by the table and blacktop area where there was no staff present. This poses a potential health and safety risk to children in care.

Based on observation, LPA observed around 10:29AM that a child was in the restroom. The staff had stepped out of the restroom area, then returned. Restroom has half-walls, so no staff is able to visually supervise the child, which poses a potential health and safety risk to children in care.

Testing results with fractional ppb readings of 0.5 ppb or greater shall be rounded up to the nearest whole number, before comparing to the Action Level. A result with values of 5.5 ppb or greater shall be deemed an Action Level Exceedance. 1 By the POC date: Facility will provide proof of repairs and retest results by 5/01/2023. Identified fixtures are not to be used until cleared for use after retesting. Facility submitted that there is an exceedance of about 11 (ppb) sample (D) room K3 from the sink faucet and 5.8 (ppb) sample (P) room 7 sink faucet. 8
