Facility self reported that a child was found by a teacher inside a classroom alone without adult supervision while door was closed.

101229 (a)(1) that no child(ren) shall be left without the supervision, including visual supervision, of a teacher at any time, except as specified in sections 101216.2 (e)(1) and 101230 (c)(1). 1 As per the LIC 624 dated 12/14/2023, Plan of Correction (POC), the involved Staff was given disciplinary warning and was reminded of the supervision guidelines. Also, C1's authorized representatives were informed of the incident. Based on LPA Estoesta Staff Interview that C1 was left on the gated playground unsupervised during a transition from the outdoor play area to the classroom, on 12/12/23 at approximately 11:45 am which is an immediate health and safety risk to children in care. 8 LPA recommended that the Directors should conduct a Staff Meeting to remind supervision guidelines. LPA will endorse the facility to the Technical Support Program (TSP) which is the non-enforcement service of CCLD and will informed the assigned Resource and Referral Agency for this facility.

Based on record review, the facility has one outlet outside that have an ALE of 5.5ppb or greater, which poses a potential health and safety risk to persons in care.
