Based on observation and record review, S1 was not have criminal record clearance associated to the facility, therefore the licensee did not comply as cited above which is a poses a potential health and safety risk to persons in care.

Based on observation, LPA observed a large black stain on the infant room floor and a carpet floor tile in the pathway in a preschool classroom lifting significantly on the corner, the licensee did not comply with the section cited above in which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on observation and interview, the changing station had a fabric cover and the area was not being cleaned and disinfected between changes, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

During visit on 07/16/2025, LPA observed a common towel in the restroom of the Love Classroom that teachers confirmed was being used by children to dry their hands which poses a potential health, safety and/or personal rights risk to persons in care. During visit on 08/21/2025 LPA observed that towel had been removed from the classroom and paper towels observed in paper towel dispenser.

Staff do not ensure bathroom sinks have disposable paper towels.

Unqualified staff are supervising day care children.
