This requirement is not met as evidence by 1 Per Director, Training has been provided to staff and S2 is no longer employed with the faciity. During this inspection, LPA obtained a copy of the staff recent trainings. Per Director, staff S2 was present 08/16 when the incident occurred and disclosed to the director that she did not inform RP of the incident the same day. RP was not informed of the incident until 08/18/2022 when S2 provided written incident report. This poses a potential risk to the health and safety of children in care. 8
