(a) The licensee shall provide care and supervision as necessary to meet the children's needs. (1) No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. Facility staff left child unattended and unsupervised in the classroom restroom for approximately 10 minutes. 1 Director states facility will be taking the following steps: (1) Teachers will conduct a head count of children during group activities and/or moving throughout the facility to and from classrooms. (2) Teachers will document the head count and keep a log and submit proof via emai to Licensing Program Analyst Bill Billones at bill.billones@dss.ca.gov

(b) Testing results with fractional ppb readings of 0.5 ppb or greater ... the Action Level. (1) A result with values of 5.5 ppb ... shall be deemed an Action Level Exceedance. Facility has taken the following steps: Shutoff and labeled the two (2) outlets and will notify parental notification of the water testing results and are reviewing options to correct the ALE. Facility has until 11/12/2022 to correct the deficiencies or have a reasonable plan to correct them and notify CCL. Facility conducted water lead testing and had two (2) outlets exceeded the action level exceedance per regulations. This poses a potential risk to the health and safety of the children in care 8

observation, interview and review of medication and child #1's file which revealed medication was accepted in plastic bag with child's name and no prescription on file. This poses an immediate risk to children in care.
