, on 4/23/2024 at 3pm, there 14 preschool children, 9 toddlers and 2 (S1 and S2) Teachers. On 5/10/2024 at 3pm, Hele Qiu stated that she was on break and S3 was supervising the toddlers with S2. S2 and S3 are Teacher Aide's. Based on LPA's Staff interview, Staff confirmed the mentioned incident happened on that date. , As a result of a Complaint Investigation dated 05/10/2024 at 11 am, LPA asked Helen Qiu to provide Children's sign-in and sign-out sheets with the signatures required by Section 101229.1 records from the Month of April 2024.
