Responsibility for Providing Care and Supervision No child(ren) shall be left without the supervision.... at any time, ...., Title... 101229(a)(1)Based on interviews and records reviewed, the licensee did not ensure that a child remained under the supervision of a teacher at all times. 1 The director stated that supervision requirements were reviewed with staff, a written warning was issued to the involved staff member, and safety locks were installed on the exterior classroom doors. The licensee will submit documentation of the staff supervision training, On 5/27/26, a child left the playground and entered an adjoining classroom without staff awareness. This poses a potential health and safety risk to children in care. 8 including the training agenda and signatures of participating staff, to the Department by 9/24/26.

A written report...within seven days.Based on interviews and records reviewed, the licensee did not report the incident 1 The assistant director stated they will complete LIC 624, Unusual Incident/Injury Report, regarding the incident to the Department. The director will review the reporting requirements with all staff responsible for reporting incidents. The facility did not report the incident to the Department as required. This poses a potential health and safety risk to children in care. 8 Proof of correction, including the completed LIC 624 and documentation of the review with staff signatures, will be submitted to the Department by 9/24/26.

Director states the protocols outlined in the facilities discipline policy will be followed.

Based on interviews and review of documentation, the facility has instituted a policy to wake children up after the scheduled nap, regardless of whether the child wishes to awaken. This is a potential risk to the health and safety of the children in care. 8 Director will also notify parents of the change in the admission agreement, Then will send proof of the parent notification to LPA by the plan of correction due date.
