Based on observation, there were paint chips in the sand and four pieces of broken glass in the outdoor activity area. The items were accessible therefore a hazard was present, which poses a potential health and safety risk to the children in care. Based on observation, one empty glass 1 Per facility representative daily outdoor checks are conducted. The items were removed during this inspection. Facility will conduct a staff training. A copy of the staff training will be submitted to the Department beer bottle and a pair of adult size scissors were in the outdoor activity space. The items were accessible and are not age appropriate therefore a hazard was present, which poses a potential health and safety risk to the children in care.
Based on observation and record review, the licensee did not ensure each child was accorded a safe and healthful environment when S1 did not wear a mask in the Center for 10 days following exposure, which poses a potential Health, Safety or Personal Rights risk to the persons in care.
Based on interviews and record review, care and supervision was not provided to meet 1 Per facility representative, the Center will ensure the ratios requirements are being met. On 11/21/23 the Center had a training regarding supervision, specifically not turning your back to the children, ratios, and communication. A copy of the training topics and sign in sheet will be submitted. the needs of C1 when C1 was involved in seven biting incidents between August and October 2023. In addition, an assistant was left alone in the classroom for 10 -15 minutes, which poses a potential Health, Safety or Personal Rights risk to the persons in care.
Based on interviews and records reviewed, 1 Per facility representative the facility will report unusual incidents as required. the Center failed to report C1 biting others on five separate dates. C1 was suspended for the behavior. This unusual incident threatened the physical or emotional health of C1 or the other children in care, which posed a potential Health, Safety, and Personal Rights risk to persons in care.
Based on observation and record review, 1 Per facility representative, a meeting was held on 10/10/23, with staff members to discuss qualifications and ratios. Administrative staff are conducting safety and ratio checks every 30 minutes to ensure ratios are being met. S1 was observed alone and supervising 10 children on 10/9/23. S1 does not meet the qualifications of a teacher . Therefore, the Center failed to meet the teacher to child ratio. This is an immediate health and safety risk to the persons. Civil penalties were assessed.
Based on LPA observation on October 2, 2023 between 2:30-2:45 p.m. the facility failed to meet the teacher to child ratio when one teacher was supervising 15 children. This is an immediate health and safety risk to the persons. 8 A staff training will be conducted to advise staff of the need to notify Administration once the first child wakes up from nap.
101238 (a) Buildings and Grounds: The childcare center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors. This requirement was not met as evidence by: 1 Per facility representative, an electrician will called to the Center within the next 24 hours. The extension cord will be removed. A staff training will be held. A copy of the invoice/report from the electrician along with the staff sign in sheet for training will be submitted to the Department. Based on LPA observation a damaged electrical outlet with a piece of exposed metal was accessible and an extension cord was running between two classrooms. This poses a immediate health, safety, or personal rights risk to a person in care. 8
because the play equipment on the play yard is not in good condition, child #1 got her hand stuck in one of the areas of separation of a concrete tunnel and Paramedics had to be called to free her hand.

Based on observation, the licensee did not comply with the section cited above when two containers of diaper cream (Non prescription medication) labeled "keep out of reach of children" were stored in the children's diaper bins in the bathroom of classroom 2. The diaper bins were stored in a shelving unit that was accessible to the children as the bins were located less that 35 inches off the ground, which poses/posed a potential health, safety or personal rights risk to persons in care.
Based on observation, the licensee did not comply with the section cited above when peeling paint was observed on the wall in the dining area, LPA observed the white sink in classroom 4 had brown residue on the sides and base of the bowl. The faucet in the bathroom of classroom 4 was leaking. The ceiling in room 3 was leaking drops of water into the light fixture and onto the floor, which poses/posed a potential health, safety or personal rights risk to persons in care.
Based on observation, the licensee did not comply with the section cited above when a cabinet in the in laundry area which held laundry soap, bleach, Ajax, dishwashing soap and additional disinfectants, was not equipped with a child safety lock and items were accessible. LPA observed the children walk past this unlocked cabinet under the supervision of a teacher on the way to the dining area for lunch. This poses/posed a potential health, safety or personal rights risk to persons in care.
Based on observation, the licensee did not comply with the section cited above when there were two wood boards with nails/screws with a sharp edges. LPA observed three empty soda cans, a spray paint can, dried palm fronds, prickly weeds, papers, empty chip bags, a used sucker, and additional debris to the back of the outdoor play space. All items were accessible to the children in care. which poses/posed a potential health, safety or personal rights risk to persons in care.
Based on confidential interviews, the staff made statements to the children such as "your ears are not working”. These comments made the children feel as though something was wrong with their ears and did not provide the child with dignity. This posed a potential Health, Safety or Personal Rights risk to the persons in care.

This requirement was not met as evidence by: 1 Per facility representative the center completed all necessary corrections by June 1, 2022 and documentation was sent to CCL. In addition, staff are required to complete a daily inspection of their classroom and daily inspections are being conducted of the outdoor play space. No further action required Based on LPA observation on 5/31/22 the toilet paper holders were not installed properly, a damaged electrical outlet was in use, molding was falling off the wall of the bathroom and an old sink and metal pipes were in the outdoor pay space. This poses a potential health, safety, or personal rights risk to a person in care. 8
101239.2 (a) Drinking Water: Drinking water from a noncontaminating fixture or container shall be readily available both indoors and in the outdoor activity area. This requirement was not met as evidence by: 1 Per facility representative, staff will be reminded to take drinking water outside. A staff training will occur and documentation will be submitted to CCL. Based on confidential interviews children drank directly from the bathroom faucet while engaged in outdoor play.
