Director will submit an attendence sheet with Site Supervisor signitures acknowledging full understanding of the content that was reviewed by June 10, 2025 (C)Any unusual incident or child absence that threatens the physical or emotional health or safety of any child. Based on interview and record review, the licensee did not report two (2) unusual incident reports to the department by the next business day as required which poses a potential Health and/or, Safety and/or, Personal Rights risk to persons in care.
