Based on record review, the licensee did not comply with the section cited above where 2 staff did not have Mandated Reporter Training Certificates which poses a potential health, safety or personal rights risk to persons in care.

Based on record review the licensee did not comply with the section cited above staff were missing MMR, DTAP, TB, and Flu vaccines or declarations which poses a potential health, safety or personal rights risk to persons in care.

Based on observations, interviews conducted, and records reviewed, it was revealed that C1 sustained an injury resulting in a bruise on their cheek while in care at the facility that could not be explained by staff which poses an immediate health, safety, or personal rights risk to children in care. 8 Director agreed to provide LPA with signatures from staff for verification by close of business on 01/12/2024.

The facility began implementing a diaper changing log on 9/26/22, as well as using alarms in the Brightwheel application for every 30-45 minutes to signal teachers it is time for diaper changes. Director will also add this implementation to the parent handbook On 9/19/22 video evidence revealed that C1 did not have a diaper change for approximately 5 hours. The video also showed the teacher's failure to wash hands in between or after diaper changes. This is a potential health and safety risk to children in care.

Based on LPA observations and photographs it was determined that the facility bathrooms were dirty and stained. This is a potential health and safety risk to children in care.

Neglect/Lack of Supervision - Child sustained an unexplained bruise while in care

Staff did not change child's diaper as often as needed.

Staff did not wash their own hands after each diaper change.
