Based on LPA's interviews, Based on LPA's interviews, 1 aide was providing care/supervision for 13 preschool children as there was no qualified teacher available. This poses an immediate health & safety risk to children in care.

Director states that T1 was immediately terminated. Director also states that they will provided training to staff on how to work with children with disabilities including engagement, feeding and communication. Training lesson plan will be submitted by POC date and submitted to CCL. Training sign in sheet submitted to CCL by 10/15/22. C1 was sitting on the edge of their mat when T1 picked them up by one (1) arm & sat them at a table. While at the table, T1 wrapped their arms and legs around C1’s arms and legs and attempted to feed C1. C1 attempted to wiggle, shake and kick their way out of T1’s hold. This poses an immediate personal rights risk to children in care.

Mrs. Johnson placed staff on administrative leave beginning day of the incident. Staff returned on 9/20/21. Mrs. Johnson went over the Personal Rights regulation with staff. Staff agreed to take online course with Smart Horizons that covered "Conflict Management for Children" and "Guiding Children's Behavior in Childcare settings". LPA and Mrs. Johnson reviewed a video showing what occurred between staff and child. In the video, it was observed that the staff grabbed the child by the shoulder after child bit staff. This left a mark on the child's shoulder. 8 Mrs. Johnson also met with the child's parents to discuss child's biting issue. Parents are working with the child at home and are addressing his challenging behavior. Child has not bit since this incident.
