Based on interviews and records reviewed, the facility did not comply with the section cited above in that two (2) different children were left unattended on two (2) separate occasions. This poses an immediate health, safety, and personal rights risk to children in care.
Based on interviews and records reviewed, the facility did not comply with the section cited above in that a child absence documented on 06/16/2026 was not reported to the department within the required timeframe, which poses a potential health, safety, and personal rights risk to children in care.
Based on interviews and records reviewed, the Licensee did not comply in immediately notifying the child's authorized representative when the child sustained a serious injury on 4/24/25 at 4:16 pm and notified at pick up time at 4:34 pm, which poses a potential Health and Safety risk to persons in care. there will be staff trainings and will include training on reporting to the department and authorized representative. Warrens stated that she will submit a list of training topics and staff sign in sheets to the Department. Warrens stated she would like to set up a visit with Technical Support Program to help support and educate staff on CCLD regulations.
Based on record review and interviews conducted it was determined that the Licensee did not comply in the section cited above by not reporting the incident to the Department when C1 was injured while in care that required medical treatment which posed a potential health, safety or personal rights risk to children in care. 8 Facility Representative stated she will watch the CCL video regarding reporting and send a summary on it by 5/16/25 by email. ***This is an amended version of the report that was created on 05/13/2025***
Based on LPA record review and interviews facility did not act swiftly to ensure the children in care were accorded safe, healthful and comfortable accommodations when made aware of C1's aggressive behavior which poses a potential Health and Safety risk to children in care.
