, on the LPA Record Review that on 12/05/2024 in between 3:54 pm to 3:56 pm, C1 squeezed through the facility’s bottom fence out to the facility’s parking out. At 3:57pm, P1 saw C1 laying on the parking lot ground next to the facility’s fence, P1 got out from his car, carried and took C1 back to the facility’s front door. At this time, S1 came aware of the situation and run immediately to the front door, out to the parking lot, met P1 and C1, took C1 from P1 and brought C1 inside the facility which posed an immediate risk to the health, safety, or personal rights of children in care. 8 LPA recommended to the Director that the Licensee can request for an amendment on the facility fence with a collaboration with the Fire Department. LPA also recommended for additional Staffing during outdoor play or re-assigning toddlers play area. The Director will submit the POC proof on or by

, based on LPA’s Record Review, at one time there were 8 children inside the bathroom (including C1) by themselves and unsupervised for about 2 minutes. At that time, Staff were in front of the Staff Break Room Door and Staff appeared to be in discussion. This poses a potential risk to the health, safety or personal rights to children in care. 8 The Licensee will include Children's Personal Rights and IMS topic to the Meeting. LPA provided a video resource from the link, https://ccld.childcarevideos.org/child-care-center-operators/ and PIN 22-02-CCP BEST PRACTICES RELATED TO THE PROVISION OF INCIDENTAL MEDICAL SERVICES IN CHILD CARE CENTERS AND FAMILY CHILD CARE HOMES.

At 10:30 am, during the LPA's Staff Record Review, Staff 1 initiated Background Check Process and was not associated to the facility which is a potential risks to the health, safety, or personal rights of children in care 8 LPA advised the Director to conduct a Staff meeting, utilize watching the California Child Care Licensing Child Care Center Operators Videos and documenting the meeting. (a)In addition to the sign-in procedure requirement of Section 101226.1(b), the licensee shall develop, maintain, and implement a written procedure to sign the child in/out of the child care center that shall, at a minimum, include the following:(1)The person who signs the child in/out shall use his/her full legal signature and shall record the time of day. 1 The Director will request in writing from the children's Authorized Representative who failed to signed in on today's date, to complete the today's date Children's Sign In and Out, reminder of the importance of the Child Sign In and Out and this requirement documentation will be submitted to Regional Office on or before 09/30/2022.
