Based on water testing results and interviews, facility tested over the Action Level Exceedance at one drinkning faucet. This poses an potential health, safety or personal rights risk to children in care.

; 1 Director stated she will discuss and provide her management team and board some options of how to make the fencing appropriately inaccessible to children. Director stated she will provide LPA with an update and decision no later than 10/12/2022. LPA's observed some areas of the fencing to be shy of 5 ft., 4 out of 6 gates do not self-latch at all distances when opened, and one area exceed 2 inches from the ground to the bottom of the fence. This poses a Potential Health and Safety risk to the clients in care.
