Based on interviews child (C1) was unsupervised, left the facility and was found in the facility parking lot on Churn Creek Rd. An immediate civil penalty of $500 applies. This poses an immediate heath and safety risk to children in care 8 Parents/guardians must sign Form LIC9224 to be kept in each child's file. (within 24 hours or the next time they come in).
