Based on record review and onsite teacher’s interview, child # 1 was left unattended in a stairway during transition at least for 3 minutes, which poses an immediate risk, to the health and safety of children in care.

... 1 Facility will conduct a staff meeting discussing ratios and how to maintain a ratio of 1:12 at all times, including use of bathroom. Facility will submit signed meeting notes to CCLD by POC date. Based observation LPAs counted 41 children being supervised by 3 adults. This is an immediate risk to the health, safety, or personal rights of children in care.
(a) The licensee shall provide care and supervision as necessary to meet the children's needs. (1) No child(ren) shall be left without the supervision of a teacher at any time... Supervision shall include visual observation. This requirement was not met as evideced by: 1 Facility held a staff meeting regarding supervision and furture plans. LPA is requesting facility to create a formal outdoor supervision plan, including an outside zoning map for teachers one child was left outside alone for approximately 5 minutes after a teacher and the other students went inside. This posess an immediate health and safety risk to children in care. 8
